Description
Description — Medical Support Equipment
The Medical Support Equipment (MSE) program funds new medical capability and new technology on naval operating ships. MSE configuration management, spares, technical manuals and installations are also funded through this line item. Requirements are determined through Commander, U.S. Fleet Forces Command (USFFC) and Commander, U.S. Pacific Fleet (PACFLT), and procurement is managed by Program Executive Office Unmanned and Small Combatants (PEO USC), Program Management Support Expeditionary Missions (PMS 408). Funding is used to procure modifications or additions to the ships' allowance items that are identified within the Authorized Medical Allowance List (AMAL) and Authorized Dental Allowance List (ADAL). AMALs and ADALs are unique to specific classes or types of ship or Commands to fulfill its intended health care mission. AMAL and ADAL allowances are approved by the respective Fleet Type Commander (TYCOM) Force Surgeon with concurrence by the Commander, Fleet Forces Command Surgeon. The initiation of a change to the current depth (allowance amount) of material or addition of a new allowance for an item(s) within an AMAL or ADAL results from either a change in the scope of care a specific ship must provide in accordance with its Required Operational Capability (ROC) and Projected Operational Environment (POE) and/or a change in the standard of medical care the assigned personnel are expected to provide. Changes to AMAL and ADAL requirements are determined through Individual Allowance Change Requests, as a result of reviews chaired by TYCOM Fleet Surgeons. The Fleet Hospital/Expeditionary Medical Facility (EMF) program funds critical medical equipment to support and provide theater hospitalization capabilities (commonly referred to as Role III Care) as defined in Joint Publication 4-02 Health Services Support and the EMF ROC and POE. This funding is used to procure investment equipment that supports the standard of care for Casualty Management as determined by the DoD Combat Trauma Surgery Committee. Funds in this line item will be used to procure medical equipment such as Direct Digital Radiology System (DDRS) and Computer Aided Design and Computer Aided Manufacturing (CAD CAM) Milling Machine, Expeditionary Medical Unit (EMU), Expeditionary Resuscitative Surgical Systems (ERSS), En Route Care Systems (ERCS), and Certified Diagnostic Mobile Lab (CDML). Funding provides support the Hospital Ships (USNS COMFORT (USFFC) and USNS MERCY (PACFLT)) major systems replacement. The program is part of the standardization and life cycle management of the MERCY Class Hospital ships sustainability plan. [P40A / Direct Digital Radiology - Procurement]: Funds provided for Direct Digital Radiology systems required for advanced diagnostic imaging. This provides shipboard medical personnel the capability to digitally process, and store images thereby reducing film development time, maintenance, and storage costs. It also provides seamless integration of digital imaging with other systems used in Telemedicine, creating a more efficient process that bypasses chemical processing and provides the ability to digitally transfer and enhance images. [P40A / Direct Digital Radiology - Installation]: Funds provided for Direct Digital Radiology system installation. The systems are required for advanced diagnostic imaging. [P40A / Radiography/Fluoroscopy, Digital]: Funding provided for updated Radiography/Fluoroscopy (RAD) for USNS MERCY. These units are needed to perform specialized procedures supporting both humanitarian and combat missions. [P40A / Expeditionary Resuscitative Surgical Systems (ERSS)]: Expeditionary Resuscitative Surgical Systems (ERSSs) are the smallest standard force package of the Navy Expeditionary Health Services Support (NEHSS) family and may be employed afloat, on any vessel, or ashore to provide immediate life and/or limb saving damage control resuscitation and surgery. It is designed to support disaggregated maritime operations and operate in austere environments. [P40A / En Route Care Systems (ERCS)]: En Route Care Systems (ERCSs) are the core capability to patient movement. ERCSs ensure patient degradation does not occur during transport to the next level of care. [P40A / Certified Diagnostic Mobile Lab (CDML)]: Certified Diagnostic Mobile Lab (CDML) provide a modern, agile, and scalable capability to conduct in-theater diagnostics and screening of infectious diseases, both ashore and afloat. CDMLs provide the capability to diagnose infectious diseases in an expeditionary environment. CDMLs are on ships to assess infection and mitigate outbreak. [P40A / Production Engineering Support]: Funding supports activities used to identify, plan, resource, and acquire facilities, hardware, software, documentation, manpower and personnel necessary for planning and management of mission critical Expeditionary Medical Family of Systems components.

The FY 2026 request was reduced by $0.086 million for Advisory and Assistance Services to promote efficiencies and advance the policies of the Administration in alignment with Executive Order 14222, "Implementing the President's Department of Government Efficiency Cost Efficiency Initiative." The FY 2026 request was reduced by $0.023 million for civilian personnel to optimize the workforce in compliance with Executive Order 14210, "Implementing the President's Department of Government Efficiency Workforce Optimization Initiative." FY 2026 BUMED (BSO 18) decrease ($0.347M) supports requirements for laboratory, X-Ray, Biomedical, preventative medicine, physical therapy, operating room, intensive care, acute care, and dental equipment. FY 2026 NAVSEA (BSO 24) Deployable Medical System increase ($2.464M) includes (+$3.400M) increase for EXMED SMART Warehousing and (-$0.936) decrease for rate and other budgetary adjustments. FY 2026 PACFLT (BSO 70) increase ($0.021M) supports the overhaul and upgrade of medical equipment required for the USNS MERCY.
